Duties and Responsibilities:
Ensure referral policies and workflows. Prioritize referrals by urgency and perform within Neighborhood Medical Center protocols.
Initiate referral process and requirements including patient demographics, insurance information and clinical encounters.
Maintain ongoing tracking and documentation on referrals through the completion of the referral process, including obtaining clinical notes for closing the referral loop.
Communicate with insurance companies to initiate prior approval/pre-certification requirements.
Request new referrals/orders per need.
Follow up with patients, care providers and community partners on all referrals.
Educate and assist patients in enrolling in Medicaid, SNAP, WIC and other entitlement programs.
Identify and utilize community resources; establish relationships with medical providers and personnel.
Act as the point of contact for patients and specialists regarding questions or concerns.
Qualifications and Experience:
High school diploma or its equivalent and/or bachelor’s degree in social work or health sciences.
Previous experience in case management or referral management preferred. Team-oriented with excellent interpersonal and communication skills.
Must clear a Level 2 Background Check.
Ability to function independently.
Ability to work flexible hours.
Bilingual in English/Spanish preferred.
