Visit aviatorhealth.co to see how we’re reimagining care for families. About the Role As a Patient Advocate for Aviator, you will work with Medicare patients throughout their healthcare journey. We’re looking for a mission-driven Community Health Worker who not only understands how to support patients but also knows how to build trusted relationships inside hospitals, skilled nursing facilities, and senior-serving organizations. This role is ideal for someone who has a background in care coordination or case management, but also thrives in fast-paced, scrappy environments. You’ll be one of the first boots on the ground helping us shape our local market expansion strategy. You’ll act as both a patient navigator and local market launcher: meeting patients, calling providers, following up on care plans, and introducing Aviator to new partners. Please note that this is a 1099 role. The role is remote. What You’ll Do Learn the Aviator systems, tools, technology, partners, and expectations, while also providing your unique expertise in every interaction. Build strong, trusting relationships with Medicare patients, where listening and empathy are the foundation for every interaction. Be able to identify and prioritize Medicare patients’ needs and assist them to maintain a streamlined care continuum. Develop comprehensive patient care plans that holistically address social determinants of health, i.e. food resources, transportation access, and support at home. Build relationships with hospital case managers, social workers, and physicians. Help refine outreach playbooks, test what works in your region, and shape new workflows. Attend local team meetings and trainings. What You Bring to the Table 3+ years proven experience in care management, patient advocacy, or healthcare navigation. Deeply empathetic and understand the Social Determinants of Health alongside experience working with diverse patient populations. Has a local network of contacts at hospitals, SNFs, or senior living communities. Can be both high-touch with patients and high-impact with provider outreach. Moves fast, documents clearly, and communicates proactively.
