Working Hours :
Monday to Friday, 8:30 AM – 5:00
PM Key Responsibilities:
Conduct assessments and develop comprehensive Person Centered Support Plans (PCSP) and backup plans. Interface with Medical Directors and participate in interdisciplinary care rounds to support the development of fully integrated care plans. Engage the member’s circle of support and manage their physical health (PH), behavioral health (BH), and LTSS needs. Perform face-to-face program assessments using various tools and pre-defined questions. Apply motivational interviewing techniques for evaluations, coordination, and management of individual waivers (such as
LTSS/IDD
), and BH or PH needs. Identify members with potential clinical health care needs using tools and pre-defined processes, addressing gaps in care, and coordinating cases with the clinical healthcare management and interdisciplinary team. Manage non-clinical needs of members with chronic illnesses, co-morbidities, and/or disabilities to ensure cost-effective and efficient utilization of long-term services and supports. Document short and long-term service and support goals in collaboration with the member’s chosen care team, including caregivers, family, natural supports, service providers, and physicians. Identify members who would benefit from alternative levels of service or other waiver programs. Serve as a mentor, subject matter expert, or preceptor for new staff, assisting in formal training and process improvement initiatives. Submit utilization/authorization requests to utilization management with documentation supporting and aligning with the individual’s care plan. Report critical incidents to appropriate internal and external parties. Assist and participate in appeals or fair hearings, member grievances, appeals, and state audits.
Qualifications & Requirements:
701 certificate and CPR certification required (training available upon onboarding if needed).
Preferred:
Case management experience, bilingual skills, time management, strong organizational skills, people skills, and adaptability to multiple computer systems. Requires a BA/BS degree and a minimum of 2 years of experience working with a social work agency, or an equivalent combination of education and experience.

BA/BS degree in a healthcare-related field.
