Judge Healthcare is seeking a LTSS Service Coordinator in Dade and Monroe County, FL!
Work Schedule:
Monday to Friday, 8:30 AM to 5:00
PM Employment Period:
May 5, 2025 – November 2, 2025
Overview:
Responsible for conducting service coordination activities for a defined caseload of individuals enrolled in specialized programs. This role focuses on facilitating a Person-Centered Planning process to support individuals in achieving their preferences, needs, and self-identified goals. Key responsibilities include assessments, developing comprehensive Person-Centered Support Plans (PCSPs), addressing physical health (PH), behavioral health (BH), and long-term services and supports (LTSS) needs, and ensuring compliance with applicable laws, contracts, and federal regulations.
Key Responsibilities:
Conduct face-to-face program assessments using pre-defined tools and motivational interviewing techniques. Identify clinical healthcare needs using pre-defined processes and coordinate care with interdisciplinary healthcare teams. Manage non-clinical needs of individuals with chronic illnesses, disabilities, or co-morbidities to ensure efficient utilization of services. Collaborate with individuals to document short-term and long-term service goals and coordinate with chosen care teams, including caregivers, natural supports, and service providers. Facilitate interdisciplinary care rounds and interface with Medical Directors for integrated care planning. Mentor and support new staff members through training and knowledge-sharing initiatives. Submit utilization and authorization requests aligned with care plans. Report critical incidents to appropriate authorities, including Adult Protective Services or law enforcement, as required. Assist in handling grievances, appeals, hearings, and state audits.
Qualifications:
Bachelor’s degree (BA/BS) in a health-related field preferred. Minimum of two years of experience in social work or equivalent combination of education and experience. Strong organizational skills, time management abilities, and adaptability to multiple computer systems. Excellent interpersonal skills, with bilingual abilities preferred. Knowledge of state-specific laws and contract requirements for LTSS service coordination is essential.
