We are seeking an enthusiastic and compassionate bilingual ( English/Creole ) Care Coordinator to join our 211 Help Me Grow team. The ideal candidate will have a genuine passion for helping children and families access the services they need. The Care Coordinator, plays a crucial role in connecting families with the resources and support necessary for the positive growth and development of their children. This position responds to calls from parents, childcare providers, early educators, health professionals, and social service agencies about concerns related to child learning, developmental, emotional and/or behavioral issues with a special emphasis on those who speak Spanish Using HMG approved screening tools, such as the ASQ and other evaluation tools, conducts screenings to determine concerns related to child learning, developmental, emotional and/or behavioral issues.
Responsibilities:
Determine the need for services and provide information assessment and referrals to community services utilizing the 211 resource database. Complete developmental screenings and provide referrals to qualified children as needed. Maintain accurate records of outreach activities, and client interactions. Links callers who are potentially eligible for services to the appropriate Palm Beach County or Treasure Coast County agency based on needs expressed Participates in health fairs and community events to increase awareness and utilization of the Help Me Grow program and 211 services.
Qualifications and Experience :
Bachelor’s degree in Social Work or Associates Degree in early child development from an accredited college or university preferred; Special education / work experience in a related field may be substituted for a degree. Proficiency with Microsoft Windows Suite; ability to enter and retrieve information in databases Ability to establish professional relationships. Bilingual English/Creole.

211 Palm Beach Treasure Coast is a private nonprofit 501(c)3 agency that was started in 1971 and quickly expanded into crisis counseling and suicide prevention. The agency cultivates a caring and dynamic team to service Palm Beach County and the Treasure Coast (five counties). Additional partners provided funding to support a quality, centralized access point for health, and human services information and crisis services. 2-1-1 was designated nationally by the FCC for information and referral purposes, and it became the telephone number for the agency’s helpline. 211 Palm Beach/Treasure Coast’s mission is to save lives through crisis intervention and by connecting people to health, mental health, and wellness services 24 hours a day every day. The position : Full time day shift Monday through Friday. Occasional weekend events.
