Job Description

Care Coordinator-Fluent in both English & Spanish HealthyMe Jupiter, FL 33458 Care Coordinator Reports to:

Health and Wellness Program Manager Full Time, Hourly, Non-Exempt Summary:

HealthyMe Care Coordinator provides navigation and referral services to individuals and families who have limited resources to assess their needs and facilitate connections to appropriate community services and programs. Care Coordinator utilizes accepted screening tools to identify unmet health and social care needs (Social Determinants of Health- SDOH) and utilizes quality patient navigation skills to assure that clients are connected to the care and resources that they need. Care Coordinator assists clients in the application process and tracks outcomes and feedback related to referrals. Care Coordinator maintains up to date knowledge of community programs, services and resources and helps to identify gaps in the healthy and social care safety net.

Requirements:

Excellent verbal and written communication skills.

  • Bilingual (Spanish) preferred
  • Education- minimum Associates Degree in related field, Bachelor’s Degree preferred
  • Community Health Worker (CHW) certification a plus Computer skills including Microsoft Office Suite Familiarity with electronic medical records or client tracking software a plus 1-2 years related experience
Responsibilities:

Conducts client assessments utilizing the Unite Us/PRAPARE Palm Beach assessment tool to determine Client’s Social Determinants of Health needs. Completes client intake process including information related to residency and poverty level utilizing approved guidelines to verify client eligibility for specific programs and services. Provides navigation and coordination assistance to help clients schedule and attend medical and ancillary service appointments. Utilizes knowledge of both in and out-of-network resources and service eligibility requirements to provide a thorough offering of the most appropriate referral options. Conducts screening to identify and refer Clients to HealthyMe services and programs. Maintains necessary follow-up with Clients to assure that services were received and/or identify barriers. Adheres to Florida Department of Health policies and process to refer eligible clients to appropriate DOH participating licensed provider when indicated. Maintains updated Client information and completes detailed Client chart documentation in Athena electronic medical records system and UniteUs referral platform. Maintains HIPAA standards of privacy and confidentiality. Conducts all interactions in a culturally appropriate manner. Positively contribute to staff morale and organizational culture. Actively participate in performance improvement initiatives. Carry out other duties as assigned in support of HealthyMe’s mission.
