Sweetser’s Certified Community Behavorial Health Clinic (CCBHC) searching for a Care Coordinator!
Care coordinators provide support to individuals accessing the Community Behavioral Health Clinic [CCBHC], through supporting intake/access to care and ongoing monitoring of care and needs. Care coordinators manage ongoing coordination of care for a caseload of individuals to include facilitating ongoing collaboration among providers and linkage to needed services. This position is part of the York County Certified Community Behavorial Health Clinic, which is located in Sanford, Maine. This position will be located in Saco, but employee may need to commutebetween Sanford and Saco locations on occasion.
ESSENTIAL FUNCTIONS
Responsible for the coordination of care of individuals in the Community Behavioral Health Clinic [CCBHC].
Facilitate ongoing collaboration and communication between providers.
Supports the intake/access processes through completion of risk screening and behavioral health/SUD screenings with individuals.
Links individuals with needed resources and services and regularly ensures services are meeting needs.
Complete timely and accurate documentation of all contacts and activities utilizing an Electronic Health Record.
Completes comprehensive surveys with individuals at intake, discharge and every six months while in care.
Responsible for tracking and administering surveys as well as data entry.
Conducts primary care screening and monitoring processes and procedures. Coordinates vaccinations for Hepatitis A and B series for clients, as indicated by screening procedures.
Support individuals in completion of applications for applicable benefits to include Mainecare, non-emergency transportation resources, and other social services.
Utilize supervisory supports appropriately and participates in all team/program meetings.

Communicate ideas and opinions in a clear and constructive manner, both verbally and in writing.
Intermediate computer skills required.
Patient facing position – mental health experience 1-2 years preferred. Must be able to speak, read, and write English fluently.
EDUCATION
Bachelor’s degree in related field or completed MHRT-C
