EST Position Purpose:

The Care Coordinator II supports care management teams by facilitating services for health plan members to ensure continuity of care, high member satisfaction, and effective coordination between healthcare providers, partners, and members. The role includes telephonic and occasional home-visit outreach to members, documentation of care activities, and liaison responsibilities across clinical and community resources.

Key Responsibilities:

Conduct outreach via phone (and occasionally home visits) to engage members, review care/service plans, address questions, and provide education.
Coordinate care activities based on service plans in collaboration with providers, community partners, members, and caregivers.
Respond to member/provider inquiries related to care/service plans.
Communicate with care managers, healthcare providers, and internal stakeholders to ensure continuity of care.
Support assessments/screenings and document member care needs.
Maintain accurate and compliant member records in line with regulatory requirements.
Identify member needs and make appropriate referrals to Care Managers or community resources.
Educate members on health benefits, resources, and disease management programs.
Meet performance expectations, including completing 50-75 outbound calls per day and submitting daily productivity reports.
Participate in initial and ongoing training and maintain professionalism in a remote setting.
