Overview:
The Medical Case Manager plays a vital role in ensuring seamless coordination of medical services for clients, supporting both internal operations and external partnerships. This role involves managing patient admissions, supporting clinical workflows, and collaborating across multiple agencies to facilitate care. The ideal candidate will possess strong organizational skills, clinical understanding, and a commitment to quality patient care.
Key Responsibilities:
Conduct admission screenings and record vital signs for incoming patients Collect and process urine drug screens in accordance with clinical protocols Upload and manage lab work in the Electronic Medical Record (EMR) system Facilitate referrals within the agency and to external service providers Coordinate approvals for outside medical and behavioral health appointments Plan and arrange transportation for clients attending outside appointments Prepare and maintain monthly Excel reports on patient discharges and referral activity Manage scheduling and maintain accuracy of the EMR calendar for both admissions and internal physician appointments Submit prior authorizations to pharmacies, including for injectable medications such as Vivitrol Perform medication reconciliation as part of client care continuity Use Microsoft Teams and Outlook daily for communication, calendar management, and interdisciplinary coordination Collaborate with team members to support the efficient flow of admissions and physician visits Act as a liaison to ensure clear, effective communication among internal departments and external agencies
Skills & Qualifications:
Proficient in Microsoft Excel, Teams, and Outlook Knowledge of EMR systems and medical documentation practices Experience with patient care coordination and case management Familiarity with prior authorization processes and pharmacy communication Strong organizational, communication, and time-management skills Ability to work both independently and collaboratively within a team Attention to detail and a patient-centered mindset EOE/ Drug Free Workplace
