Job Description The Linkage to Care Coordinator is responsible for ensuring that individuals seeking Primary Care, newly diagnosed with HIV, or those re-engaging in care, are successfully linked to medical, psychosocial, and supportive services. This role involves close collaboration with healthcare providers, case managers, and community-based organizations to facilitate timely access to care and treatment, reduce barriers, and improve health outcomes. Role:

Provides immediate and compassionate support to individuals newly diagnosed with HIV
Coordinate and facilitates linkage to HIV medical care within 30 days of diagnosis or referral
Collaborates with Clinician Lead on surveillance and quality management to identify patients who are lost-to-care and facilitate means for them to be brought back into care by clinic staff
Participates in marketing interventions to community partners
Develops a protocol for identifying patients who are lost-to-care and re-engaging them back into care
Conducts comprehensive needs assessments and develop individualized service plans
Maintains regular contact with clients to monitor progress and address barriers to care
Collaborates with healthcare providers, case managers, and community partners to ensure continuity of care
Tracks and documents client progress using electronic health records
Participates in case conferences, staff meetings, and training sessions
Ensures compliance with confidentiality and HIPAA regulations
Collects and reports data for program evaluation and quality improvement
Collaborates with Avita’s Corporate Compliance department to ensure policy and procedure adherence
Additional responsibilities assigned as needed

About You:

High school diploma or GED required, Associate’s degree or higher preferred, especially in a health-related field
Minimum of 2 years of experience in HIV services, case management, or healthcare coordination
Knowledge of HIV/AIDS, health disparities, and community resources
Ability to work with diverse populations in a culturally competent manner
Excellent interpersonal and communication skills Maintains a high level of patient confidentiality
Demonstrates attention to detail and the ability to multi-task
Proficient in English, with strong verbal and written communication skills
Bilingual in Spanish preferred, however not required
Proficiency in Microsoft Office and electronic health record systems
Experience with eClinical Works preferred Must have a positive attitude, be energetic and flexible, have excellent phone etiquette, and provide exceptional customer service

Pay range:

$17.00/hr.
