Job SummaryThe Clinical Care Partner MA is responsible for coordinating pre visit patient care following established standards and practices of population health management. Monitors disease registry for patient scheduled appointments, procedures, tests and referrals while communicating high risk patient needs to registered nurse care coordinator. Relies on guidelines to accomplish tasks. Relies on some experience to complete a job. Works under close supervision.

Minimum RequirementsExperience•3 years experience as a medical assistantEducation•Graduate of an accredited medical assistant program, or 2 years additional medical assistant experience in lieu of education.
