High Point & Affiliated Organizations is a health and humanservice agency whose mission is to treat and prevent substance usedisorders and mental illness. High Point has programs located throughoutSoutheastern Massachusetts offering a full continuum of care forsubstance use and mental health treatment, including inpatient,outpatient, residential, and community-based services. Aftercare Coordinator Duties & Responsibilities:

Assist patients withidentifying aftercare and continued treatment services and supportrecovery; Assist patients by providing information regarding communitysupports, family supports, and coordination of services; Participatingin setting the therapeutic milieu of the unit, and remaining calm in acrisis; Facilitate patient groups to discuss aftercare/servicecoordination, family support, and relationships; Complete referrals foraftercare based on patient needs and identified preferences tocommunity-based supports, sober housing, continued 24hour treatmentsettings, and outpatient; Coordination of MAT services followingdischarge with identified provider, counseling, and ensure dosinginformation if effectively communicated; Advocacy regarding patient?scurrent treatment, and aftercare; Documentation of family meetings andmaintain monthly treatment; Excellent interpersonal skills to assist thestaff on the unit such as providing psych educational groups,facilitating peer led groups, and assisting with other documentation asneeded; Maintenance of proper boundaries with patients; Provideinformation on area support groups and articles on addiction; Attendsrequired meetings and mandatory trainings; Knowledge of treatmentservices, community supports, and continuum of care; Ability toeffectively communicate needs of patient with referral agencies; Otherduties as assigned by Program Director of Clinical Services.

AftercareCoordinator Qualifications:

Minimum of a high school degree orequivalent GED; Minimum of (1) year supervised counseling experience insubstance use treatment or closely related field; Prior substance abuseexperience preferred.
