Care Coordinators working for SeniorsPlus in the State of Maine assist clients to make informed choices to live safely and independently in their communities. Responsibilities Coordinates care adhering to a philosophy of person centered, strength based, proactive planning while using a collaborative partnership approach and follows his/her applicable practice and ethical standards
Completes psychosocial assessments, risk assessment, goal development, action plans, and back-up plans
Adheres to contract requirements and current state program regulations in accomplishment of daily work
Upholds the values of client respect, choice, access, dignity, quality, prevention and early detection
Works with providers to gather information about Medicare or services supported by third party Insurers
Ensures evaluation of the effectiveness of the plan of care, makes recommendations for care plan modifications and identifies barriers
Adheres to all appropriate protocol related to face to face visits (home visits), including documentation and appropriate and timely communication for follow up Qualifications Bachelor’s degree in Health or Social Services required
Minimum of one year in Social/Human/Community Services required
Familiarity with the long term care system a plus, strong computer and organizational skills required
