Memory Care Coordinator Generations Assisted Living & Memory Care 182 Chattyrob Lane, West Jefferson, NC 28694

Position Summary:

The Memory Care Unit Coordinator plans, directs and assists in all special care staffing, service needs and daily activity needs that promote the philosophy of our Memory Care Unit.

Primary Responsibilities:
Resident Health & Wellness:

1. Review all resident information and complete a resident registry of the resident within 72 hours of admission. Ensure a resident assessment and care plan is completed within 30 days of admission; within 10 days of a significant change and/or annually. 2. Alert RN to new LHPS tasks and/or changes in resident’s status which would prompt a new LHPS task sheet and/or care plan review or change. 3. Conduct a pre-admission screening assessment prior to move-in/re-admission to ensure that the residence can provide for the resident’s individual needs and regulatory requirements have been fulfilled. 4. Oversee the resident’s medication schedule for residents receiving medication administration services. Oversee medication ordering/returns from appropriate pharmacy. 5. Ensure proper documentation for medication assistance/administration. 6. Work closely with the RN to meet with each resident on a regular basis to provide health and wellness checks and answer any health related questions. 7. Maintain ongoing communication with the resident, resident’s family and physician regarding the resident’s medical needs. 8. Coordinate additional health services the resident may need. 9. Assist in coordinating resident transfer to and from a hospital or another facility. 10. Assist with counseling residents and families. 11. Be on call for questions and resident emergencies as scheduled. 12. Assist in coordinating residents’ dietary needs with the Food Service Director. 13. Review changes in residents’ conditions with the RN, Administrator and resident’s physician’s. 14. Assess residents as needed and monitor their health. Solicit support from RN as needed. 15. Ensure that all infection control guidelines are met on unit. Management 1. Manage and direct Memory Care Unit resident care staff. 2. Assist Administrator in implementing state regulations as required. 3. Complete all agency and state required documentation in a timely manner. 4. Function as part of management team (manager on duty coverage, on-call scheduling, marketing the facility, planning and participating in staff meetings and facility events). 5. Assist with resident care staffing needs. Nursing & Medication Assistance 1. Assist in monitoring all aspects of the medication administration program. Ensure pharmacy is notified of changes. 2. Ensure resident re-assessments are conducted as outlined by licensure guidelines. 3. Ensure resident care plans are developed and implemented to meet needs of residents per agency policies and state licensure guidelines. 4. Monitor physician’s orders to ensure they have been followed. Resident’s Rights 1. Assist with ensuring compliance and understanding of all regulations regarding residents’ rights at all times and report observed violations. Training and Regulatory Compliance 1. Attend meetings and training sessions as required and in accordance with state guidelines. Activities 1. Oversee daily activities with residents in the Memory Care Unit that promotes the philosophy of our Memory Care Unit (one-on-one activities, small and large group activities). 2. Work closely with the SCU Activities Coordinator to provide appropriate activities for residents in the Memory Care Unit. Knowledge & Skills 1. Associates degree in nursing or human services field preferred. 2. Minimum of 3 years of experience in CNA, Med Aide, or Human Services field. 3. Previous experience with Alzheimer’s and memory impaired individuals, experience with older adult population in long-term care setting. 4. Knowledge of and execute confidentiality according to HIPAA guidelines. 5. Ability to demonstrate positive attitude at all times, with professional demeanor. 6. Genuine interest and concern for seniors. 7. Age 21 years or older.
