LOCATION

Remote – must live in or near Rowan County, NorthCarolina. Incumbent in this role must reside in North Carolina orwithin 40 miles of the NC border. Travel is required.

GENERAL STATEMENT OF JOB

The Care Manager Extender, hereafterreferred to as Extenders, will support care managers in deliveringTailored Care Management and fulfilling the member contactrequirements. Extenders will help Vaya Care Management best meetthe needs of members, build efficient care teams by creatingadditional workforce capacity, and allow care managers andsupervisors to focus on key tasks for assigned members as well aspermit them additional time for members with intensive or complexneeds. Certain components of Tailored Care Management will alwaysbe led by a care manager. However, some functions may be performedby an Extender as part of Tailored Care Management. Extender willreport to Care Manager and remain within the scope delineated inthis job description and only performing functions within theirtraining, scope, and abilities. Extenders will participate with theCare Manager and care team in supporting the member and integratecare team’s goal in a holistic approach and document interactionsand activities within the identified administrative healthcarerecord according to departmental standards. Supervising caremanagers, care managers, and extenders will be in regularcommunication and coordinating efforts to mitigate duplicative orinappropriate outreach to assigned members. •Remote candidates mustlive in/ near the counties being served

ESSENTIAL
JOBFUNCTIONSMember Outreach, Support & Education:

Extenders maysupport care managers in delivering Tailored Care Management byperforming activities that fall within the below categories:

Performing general outreach, engagement, and follow-up withmembers.

Coordinating services/appointments (e.g.,appointment/wellness reminders, arranging transportation). Engagingin health promotion activities (as defined in the Tailored CareManagement Provider Manual) and knowledge sharing; such asproviding education on members’ chronic conditions; teachingself-management skills and sharing self-help recovery resources;Providing education on common environmental risk factors includingbut not limited to the health effects of exposure to second andthird hand tobacco smoke and e-cigarette aerosols and liquids andtheir effects on family and children; and Promoting wellness andprevention programs.

Sharing information with the care manager andother members of the care team on the member’s circumstances.

Providing and tracking referrals and providing information andassistance in obtaining and maintaining community-based resourcesand social support services. Participating in caseconferences.

Support the care manager in assessing and addressingunmet health-related resource needs.

QUALIFICATIONS & EDUCATIONREQUIREMENTS

High school diploma or GED is required for thisposition. Must be at least 18 years of age and trained in TailoredCare Management. Must meet one of the following requirements:

Be aperson with lived experience with an I/DD or a TBI withdemonstrated knowledge of and direct personal experience navigatingthe North Carolina Medicaid delivery system; orBe a person withlived experience with a behavioral health condition who is aCertified Peer Support Specialist; orA parent or guardian of anindividual with an I/DD or a TBI or a behavioral health conditionand has at least two years of direct experience providing care forand navigating the Medicaid delivery system on behalf of thatindividual (parent/guardian cannot serve as an extender for theirfamily member); orHas two years of paid experience performing thetypes of functions described in the “Duties/Responsibilitiessection of this role description in addition to one year of paidexperience working directly with the Tailored Care Managementeligible population.

Licensure/Certification Requirements:

Nonerequired. Certified PSS (Peer Support Specialist) and/ or WRAP(Wellness Recovery Action Plan) Facilitators preferred.

MENTAL/PHYSICAL REQUIREMENTS

Close visual acuity to performactivities such as preparation and analysis of documents; viewing acomputer terminal; and extensive reading.

Physical activity in thisposition includes crouching, reaching, walking, talking, hearingand repetitive motion of hands, wrists and fingers. Sedentary workwith lifting requirements up to 10 pounds, sitting for extendedperiods of time. Mental concentration is required in all aspects ofwork.

KNOWLEDGE, SKILL

& ABILITIESExtenders must complete the fullTailored Care Management training curriculum. Additionally, toensure Extenders are sufficiently prepared and capable to performtheir duties, extenders’ training must include practical(“hands-on”) training modalities and evaluation, which may includerole play, use of call scripts, and practice sessions.

Tailored Plan eligibility andservices Whole-person health and unmet resource needs Components ofHealth Home/Tailored Care Management Ethics, boundaries, andpersonal safety Building a trusting relationship CommunityintegrationHealth promotion I/DD- and TBI-specific trainings (ifworking with members from these communities) Transitional caremanagementSkills to support health behavior change, includingmotivational interviewing Person-centered needs assessment and careplanning, including LTSS and other needs Preparing members for andassisting them during emergencies and natural disasters Infectioncontrol and prevention practices Understanding virtual/telehealthapplications Needs of justice-involved individuals Additionaltrainings for serving:

ChildrenChildren with complex needsPregnantand postpartum persons with substance use disorder (SUD) or withSUD history Members with LTSS need Extenders must also demonstratethe following skills and abilities:

Ability to establish appropriateand respectful relationships/ partnerships with persons with a widerange of disabilities and healthcare conditions and theirfamilies.

Ability to interact professionally and effectively withpersons who are upset and who disagree.

Ability to express ideasclearly and concisely orally and in writing.

Ability to work as ateam member and under the direction of the Care Manager and theCare Management department.

Good customer service skills areessential.

RESIDENCY REQUIREMENTS

The person in this position ISrequired to reside in North Carolina or within 40 miles of the NCborder.

SALARY:

Depending on qualifications & experience of candidate. Thisposition is non-exempt and is eligible for overtimecompensation.
