Personal Care Coordinator Vital Homecare Hickory, NC 28601
Job Summary:
Vital Homecare is seeking a Patient Care Coordinator/Scheduler who is reliable, organized, and driven to provide excellent support to both patients and caregivers. This full-time, in-office role is essential for maintaining smooth operations, accurate scheduling, and timely coverage across all patient cases.
Key Responsibilities:
Answer incoming phone calls promptly and professionally, minimizing missed calls. Manage, adjust, and confirm caregiver schedules to ensure full case coverage.
Respond to caregiver call-outs by:
Contacting the patient to confirm if a replacement is needed. Making every effort to cover the shift. Maximize caregiver visit efficiency. Monitor the call dashboard per agency policy and document all missed clock-ins/outs. Notify patients and vendors promptly of any service changes. Document all missed visits accurately with appropriate reasons. Enter disciplinary notes into the Discipline Smart Sheet . Record incidents or service changes in the Incident Smart Sheet . Update changes in patient status (e.g., hold, discharge, hospitalization, resume, increase/decrease) via the Interim Order Smart Sheet . Escalate patient complaints to the Administrator immediately. Submit daily coordination reports as directed. Support additional administrative duties due to staff absences or high-volume demands. Ensure all tasks are completed before the end of the workday. Perform other duties as assigned.
Job Requirements:
Prior experience in home care coordination or healthcare staffing strongly preferred . Strong phone and interpersonal communication skills. Excellent multitasking and organizational skills in a fast-paced environment. Ability to take initiative and work independently while managing time effectively. Detail-oriented with strong documentation and record-keeping ability. Basic computer proficiency; capable of learning agency software systems quickly. Must follow agency protocols and policies (training provided).
Preferred Skills:
Proficiency in medical administrative tasks including scheduling and documentation. Working knowledge of homecare processes and medical terminology.
