Join our rapidly growing network of 37 practice locations across the Triangle Area (Raleigh-Durham-Chapel Hill), the Charlotte Region, and Wilmington, NC. At Avance Care, we are committed to transforming healthcare. As one of North Carolina’s largest networks of independent primary care practices, we provide comprehensive services designed to support the physical, mental, and emotional well-being of our patients. As a Referral Coordinator, you will play a vital role in ensuring continuity of care. You’ll assist providers by promptly submitting referrals to high-value specialists and closing the loop of care by obtaining consult notes after specialist visits. This full-time role consists of 8-hour weekday shifts, with no weekend work required. The position follows a hybrid schedule, with 3 days remote and 2 days onsite. Our comprehensive benefits package becomes available on the first of the month following 30 days of employment.

Selected Responsibilities:

Ensure referrals are submitted, accepted and followed up on in a timely manner Provide appropriate clinical information to outside entities, as needed Ensure complete and accurate registration for patients Assist patients in problem solving issues related to their referrals including taking an advocate role on the patient’s behalf with the carrier to ensure approval of necessary services in a timely fashion Maintain detailed and accurate referral documentation and communication with referring providers within the electronic medical record Establish and maintain relationships with providers and outside entities Review details and expectations about the referral with patients as needed Ensure patient’s chart is up to date with consult information from other entities Monitor and ensure proper utilization of the High Value Specialist and Medical Neighborhood networks so patients have access to the right care at the right tim Communicate effectively with specialist offices assuring safe and coordinated transitions of patient care Candidates should possess a High School Diploma or, preferably, an Associate’s degree in healthcare administration or a related field. Familiarity with medical terminology and at least one year of relevant experience are preferred.
