Modern office setting Job Summary Perform general outreach, engagement, and follow-up with members. Coordinate services/appointments (e.g., appointment/wellness reminders). Engaging in health promotion activities (as defined in the Tailored Care Management Provider Manual) and knowledge sharing. Sharing information with the care manager and other members of the care team on the member’s circumstances Provide and track referrals and provide information and assistance in obtaining and maintaining community-based resources and social support services. Support the care manager in assessing and addressing unmet health-related resource needs. Assist individuals in making informed choices regarding their care and services and other life decisions affecting their illness. Facilitate person-centered, effective, positive relationships with people receiving services using positive approaches that promote self-determination in all areas of life. Assist in the development of the goals in the tailored plans and comprehensive crisis plans to assist individuals to progress to goal completion. Provide and/or arrange transportation for people receiving support as required. Attended all required meetings related to care planning and provider engagement. Notify Care Manager of any new service needs identified during service monitoring. Participate in case conferences. Build partnerships with public and private mental health agencies, child serving agencies, DSS, DJJ, and community partners, to develop programs and resources to increase family involvement in care delivery. Link individuals to appropriate community-based services to ensure that they are actively engaged in the service, including supports related to housing, employment, and nutrition. Maintain accurate caseloads and tracking tools to ensure correspondence is being mailed to identified individuals within the necessary timeframes. Maintain medical record compliance/quality. Ensure timely documentation of Care Coordination activities as required by department policy and procedures.

MINIMUM REQUIRED EDUCATION

Is at least 18 years of age High School Diploma or equivalent

MINIMUM REQUIRED EXPERIENCE

Has two years of paid experience performing the types of functions described in the “Extender Functions” sections below, with at least one year of paid experience working directly with Tailored Care Management eligible population. A parent or guardian of an individual with I/DD or a TBI or a behavioral health condition and has at least wo years of direct experience providing care and navigating the Medicaid delivery system on behalf of that individual (parent/guardian cannot serve as an extender for their family member). or Be a person with lived experience with an I/DD or a TBI with demonstrated knowledge of and direct personal experience navigating the Medicaid delivery system. or Be a person with lived experience with a behavioral health condition and a Certified Peer Support Specialist.
