T he care Coordinator role provides professional care, under nursing and physician supervision to patients of an assigned physician team. Provides an accurate collection of patient information to facilitate the care of patients based on a priority of need. Qualifications and Experience High school diploma or equivalent Medical training from an approved program preferred At least two years of experience in a physician’s office, hospital, or equivalent setting Basic knowledge of medical terminology Working knowledge of managed care basics, and experience working with a managed care and Medicare population Comfortable using email and interacting with Internet applications Knowledge of practice management and word processing software Excellent communication skills Excellent documentation skills Responsibilities include, but are not limited to, the following Triages incoming telephone calls according to established policies and procedures Schedules/reschedules appointments with physician for established patients Prioritizes patient needs based on collected information obtained through patient interviews or physical examination Instructs/directs patients in a timely manner according to established policies and protocols related to plan of care Keeps an open line of professional communication with assigned staff and physicians regarding patient needs Maintains a pleasant and cooperative demeanor while performing all duties Ensures documentation in medical record accurately reflects communication and patient’s plan of care Telephones prescription requests to pharmacy per physician order; documents action in patient medical record Performs patient education when appropriate including oral and written education related to diagnosis, procedures, and/or medications Make changes in provider schedules as directed by physician or management Provides feedback for the development and updating of written policies, procedures, and protocols in conjunction with the Clinical Manager and Team Lead Assists with orientation for new staff and ongoing training for department staff as directed by Clinical Manager and Team Lead Maintains documentation of Triage statistics as directed by
