What We’re Looking For Trillium Health Resources has a career opening for TCL Transitions Complex Care Unit Manager. This position provides oversight to ensure members receive whole person integrated care, to facilitate positive outcomes for members including input for complex care coordination and provision of oversight to staff to ensure members receive appropriate assessment, discharge/transition planning, and linkage to services in the community. The ideal candidate has experience in TCL and understands this mobile position allows work to be done in a variety of locations including facilities, members’ homes and in the community. Apply for this amazing opportunity today! On a typical day, you might: Identify needs with behavioral/mental health, physical health, co-occurring, co-morbid or multi-morbid conditions. Collaborate with internal staff across disciplines/teams (Care Coordinators, Clinicians, OT, COTA, Housing Specialists, Peers, etc.) to facilitate integrated care. Monitor the Care Plan (physical, behavioral health and social determinant concerns), service delivery and health and safety of the members. Perform clinical functions of discharge/transition planning and diversion including clinical interviewing; obtaining and reviewing clinical records; identifying potential treatment needs; assessing barriers to treatment and recommending solutions; and assessing general health needs and recommending referrals. Oversee collaboration between internal and external staff to coordinate discharge planning from Community Hospitals for primary medical admission. Oversee staff who assist with Residential Screening Verification Process (RSVP) Screening to determine independent living needs of members. Ensure completion of Complex Care Nursing Assessment and/or functional assessment as well as additional assessments as needed to identify and link members to appropriate services/supports. Participate in Integrated Multidisciplinary Care Teams. Review medical history and assessments.

Bachelors in

Nursing Preferred Education:

Advanced degree preferred.

Experience:

Bachelor’s degree with a minimum of one (1) year of documented experience working with the co-occurring of physical health and behavioral/mental health, physical health, or chronic condition population. OR Equivalent combination of higher education/experience.

Preferred Experience:

Experience in all three (3) areas listed above preferred. Supervision of two (2) or more employees preferred.

License/Certification:

Fully licensed by the North Carolina Board of Nursing as a Registered Nurse (RN). Must have a valid driver’s license
