Job Summary:
The service coordinator ensures patients’ access to services and community resources through effective coordination and collaboration with staff within the agency as well as with community partners.
Essential Duties/Responsibilities:
Provides oversight and supervision of the Adult Services Program Case Management Team, which includes Targeted Case Managers, Community Case Managers and Benefit Specialist Responsible for providing monthly case management services. Monitor each staff’s caseload, provision of services and completion of documentation. In conjunction with the Assistant Director, manage day to day case management concerns that arise via staff or patients. Provides Critical Time Intervention (CTI) services in line with the CTI fidelity. Communicate and collaborate on treatment planning and care coordination amongst treatment team members to ensure patients are receiving quality care and are linked to needed resources to maintain independence in the community. Will work in conjunction with the Care Coordinator in following duties: coordinate hospital discharge planning of patients to include collaboration with hospitals and obtaining necessary documentation, and agency to agency transfers. In a timely manner, document activities, service plans, and required metrics in the prescribed electronic health record, as identified by the agency Other duties as assigned.
Required Skills/Abilities:
The individual must possess the knowledge, skills and ability noted below or be able to explain and demonstrate how the individual can perform the essential functions of the job, with or without reasonable accommodation, using some combination of skills and abilities. Must be able to establish a rapport with the patients, and treat all patients with dignity and respect. Ability to organize work-load so that clinical and administrative duties are completed in a timely and thorough manner. Must be able to work independently. Understands the LRMHC mission and applies this knowledge productively to shape internal policies and external contacts. Must be capable of working closely with other agency staff to expand the knowledge of mental health services. Strong verbal and written communication skills, and have the ability to read and analyze data and reports. Must be comfortable technology. Attendance is a requirement of this position.
Education and Experience:
Bachelor’s Degree in Social Work, Counseling, Psychology or related field preferred or required with 2 to 3 years’ experience of supervisory experience preferred
Physical Requirements:
Is able to sustain light physical effort such as frequent standing, walking, bending, stooping, or reaching and/or occasional lifting of items 20 lbs. or more. Is able to tolerate periods of heavy workload or excessive stress
Salary:
This position has a pay range from $49,000-$52,000 per year
