Job Description Works incoordination with interdisciplinary teams to organize, prepare, anddeliver evidence based clinical services across the continuum ofcare. Maintains ethical principles and professional standards ofpractice in adherence to the discipline’s professional code ofethics. • Performs clinical diagnosis in conjunction withconducting a comprehensive bio-psychosocial assessment andestablishment of a care plan. • Champions integrated care modeltreatment guidelines in the primary care setting by providingapproximately 6-8 sessions of short-term, focused treatment.

Qualifications Master’s in social work or related field requiredLCSW, LPC, LMFT, or LADC required Prior experience working in amedical setting preferred. Bilingual preferred.
