Job Description
Overview Provides content expertise in end-of-life care, honoring care decisions (HCD) and advance care planning (ACP) to DH-H under the direction of the VP Geriatric Center of Excellence. Establishes and maintains ACP as a standard of care through consultations, training, education and electronic health record embedded tools and resources. Provides leadership for system wide implementation of patient engagement strategies in ACP. Responsibilities Promotes ACP as a standard of clinical care across the health system. Identifies and engages key stakeholders. Coordinates ACP implementation, optimization and integration with the care model across clinic, hospital, home, nursing home and community settings and across clinical service lines. Works closely with leadership to develop best practice in meeting capacity assessment requirements, including education, workflows, documentation, and measurement and reporting. Acts as an advisor to the Aging Resource Center. Trains staff in ACP conversations. Works with medical, nursing and ancillary staff to educate, implement, and evaluate the effectiveness of advance care planning principles in patient care.5. Acts as consultant for clinicians and administrative staff looking to integrate ACP tools into workflow. Acts as an advisor to staff, patients, families and other community partners on end-of-life care and advance care planning including protocols, policies and ethical guidelines. Develops and maintains and reports on a meaningful data set that includes patient and clinician utilization of SDM tools, as well as satisfaction. Identifies areas of opportunity and leads projects/initiatives to improve and spread best practices to insure that patients and their families have information about risks and benefits of treatment and participate in their health care decisions. Advocates for HCD best practice and represents D-H in state and national venues and organizations. Partners with stakeholders across the system to establish ACP goals and to achieve results. Proactively addresses issues in order to keep the work on track. Monitors, measures and reports on program progress. Develops and delivers competency training for individuals in the system that are counseling patients in end-of-life care decisions, identifying appropriate resources needed to provide ACP services for all stages of advance care planning from healthy adults through those nearing the end of life. Ongoing mentoring of facilitators and instructors to ensure quality, consistent ACP service is delivered. Facilitates ACP conversations with patients and community members as appropriate. May provide individual decision support and facilitates group classes. Performs other duties as required or assigned. Qualifications RN or Bachelor’s Degree in an Allied Health profession, Public Health, Health Administration or related field required. Master’s Degree preferred. Nursing degree preferred. 5 years health care management experience with work in shared decision-making, advance care planning, chronic disease management, care coordination, bereavement and/or palliative care preferred. Ability to interact effectively with high-level internal and external contacts including physicians, administrators and executives required. Demonstrates an ability to work independently and with diverse teams; manage projects, solve problems, make decisions, effectively resolve conflict, solid clinical and ethical judgment. Excellent organizational and time management skills with an ability to manage multiple priorities. Excellent computer and data management skills. Demonstrated success in securing external support for research project and/or a health program preferred. Required Licensure/Certifications None
