We are looking for a Care Coordinator II (Case Manager) to join our team! This something-new-every-day job is for someone who wants to make a difference in the lives of veterans by providing case management. In this role, you will have the opportunity to work with and have a positive impact on services members, veterans and their families (SMVF). You will develop rapport and engage in a holistic client-driven plan for stability, facilitate connection to long-term benefits and services that are deserved and earned. You will work to establish landlord relationships, identify housing opportunities and access to resources that will help address needs such as substance misuse, obtaining healthcare, mental health care connections, housing support, veteran benefits, financial resources, such as money management, and applying for
SSDI/SSI.
Skills needed to be successful in this role: Ability to meet clients ‘where they are at’ and develop rapport despite presenting challenges Attention to detail and ability to navigate databases and systems to retrieve and enter critical data to ensure your clients are enrolled in all deserved programs Quick response time and ability to prioritize on the fly Flexibility in the when and the how of service delivery Confidence working independently, using sound judgment on your need for consultation and guidance Effectively determine the needs of your veteran clients and balance home-based and community-based service delivery Demonstrated passion for and commitment to collaborating with other providers to ensure prompt, non-duplicative and productive service delivery Ability to effectively handle crisis situations, providing support and resources as needed What you’ll need: Master’s degree in human services or related field (counseling, social work, social and behavioral sciences, education, human development, mental health and human services, psychology, psychology/educational psychology, and sociology) with two (2)+ years relevant experience, or Bachelor’s degree with at least five years related experience as specified under Essential Responsibilities. Experience in navigating and coordinating benefits / services preferred. Understanding of military structure and culture preferred. Prior case management experience helpful. Ability to travel statewide and complete outreach walks through encampments with community partners
Hours:
Full time 40 hours position. Work will be performed in the community, from home, and within local office (Manchester).
Compensation:
$26.10 to $30.60 per hour, depending on experience and qualifications.
