Job Summary Responsible for providing administrative support to the case management team, facilitating effective patient care coordination, and ensuring efficient workflow within the department. This role is essential for enhancing communication among healthcare providers, patients, and families. Does this position require Patient Care?

Yes Essential Functions

  • Assist case managers with the coordination of patient care services, including scheduling appointments and follow-ups.

Maintain accurate and organized patient records, ensuring compliance with confidentiality and regulatory standards. Prepare and compile necessary documentation for case reviews and meetings. Communicate with patients and their families to provide information on services, resources, and discharge planning. Collect and input data into electronic health record (EHR) systems to support case management processes. Qualifications Education High School Diploma or Equivalent required and Associate’s Degree Healthcare Administration preferred or Associate’s Degree Related Field of Study preferred Can this role accept experience in lieu of a degree? Yes Licenses and Credentials Experience Experience in a healthcare administration, clinical office operations or related role 2-3 years required and Experience in case management 0-1 year preferred Knowledge, Skills and Abilities
