The Care Manager will be responsible for ensuring that clients receive comprehensive support tailored to their unique needs. Duties Assess client needs and develop individualized care plans that address their specific circumstances. Collaborate with healthcare providers, social services, and community organizations to coordinate care effectively. Utilize motivational interviewing techniques to engage clients in their care process and encourage positive outcomes. Manage crisis situations by employing effective crisis management strategies to ensure client safety and well-being. Maintain accurate documentation of client interactions, care plans, and progress notes in compliance with organizational standards. Experience Experience working with diverse populations and understanding the unique challenges faced by clients in various circumstances. Ability to demonstrate effective communication skills, both verbal and written, while maintaining confidentiality. College degree is required, preferably in the social sciences.
