Nursing Full Job Description Job Summary:

We are seeking a dedicated and compassionate Care Coordinator to join our team. The Care Coordinator will serve as a liaison between patients, their families, and healthcare providers to ensure that patients receive the highest level of care and support. The ideal candidate will have a strong background in medical coding, discharge planning, and case management.

Duties:
  • Advocate for patients’ rights and ensure they receive appropriate care
  • Collaborate with healthcare providers to develop individualized care plans
  • Assist patients and their families in understanding medical procedures and treatment options
  • Coordinate discharge planning and ensure smooth transitions between healthcare settings
  • Provide emotional support to patients and their families during challenging times
  • Conduct assessments to identify patient needs and develop appropriate interventions
  • Collaborate with interdisciplinary teams to ensure comprehensive patient care
  • Provide education on disease management and prevention strategies
Experience:
  • Minimum of 2 years of experience in case management or patient advocacy role
  • Experience working with diverse patient populations, including pediatrics and behavioral health
  • Familiarity with acute care settings and the ability to navigate complex healthcare systems
  • Excellent communication skills, both written and verbal
  • Ability to work independently and as part of a team
  • Compassionate and empathetic approach to patient care
Education:

Associates degree in Health, Human Services, Social Work, Mental Health, or RN or LPN credentials and 2 years of qualifying experience. Plus a CASAC-T certification, required. Bachelor’s Degree in social work, psychology, nursing, rehabilitation, education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, sociology, speech and hearing or another human services field; Valid Driver’s License a must If you are passionate about making a difference in the lives of patients and their families, we encourage you to apply for this rewarding position.
