Description
We are seeking a compassionate and skilled Physician Assistant or Nurse Practitioner to join the Palliative Medicine and Supportive Care Department. As a member of the inpatient consult service team, this role plays a critical part in providing patient- and family-centered care to individuals facing serious illness. You will contribute to the assessment and management of complex symptoms, help facilitate meaningful goals-of-care conversations, and support the needs of patients and their loved ones.
Job Duties include;

• Assess and respond to the physical, emotional, and psychosocial needs of
  patients receiving palliative care.
• Collaborate with interdisciplinary team members to manage complex
  symptoms and enhance patient comfort.
• Participate in and lead discussions related to goals of care, advance care
  planning, and decision-making support.
• Coordinate care plans and ensure continuity across settings.
• Document assessments, interventions, and patient outcomes thoroughly and
  accurately. Maintain accurate, complete, and timely documentation in the
  electronic medical record (EMR) of all patient encounters, treatments, and
  outcomes.
• Provide patient and family-centered care that is compassionate and effective.
• Collaborate with physicians, social workers, and other healthcare professionals
  to provide comprehensive and coordinated care.
• Engage in continuous learning and professional development to keep current
  with advancements in medical practice, ensuring the provision of high-quality
  care.
