Anniversary Bonus & Celebration We are currently seeking a dedicated and passionate Memory Care Coordinator.

Memory Care Coordinator Responsibilities include:

Supervise, develop, and schedule all resident care staff in memory care according to PSL policies, procedures, and standards Perform evaluations and reevaluations to determine appropriate residency, resident needs, and then assist in establishing appropriate service plans for memory care Orient and assist all residents in adjusting to life in the community in memory care Act as a liaison for all aspects related to resident care in memory care within the community Responsible for all documentation completed by resident care department staff in memory care, including 24-hour logs, care task charting, toileting schedules, and all other documentation On-call coverage as needed

Memory Care Coordinator Education & Experience:

Licensed Practical Nurse (LPN) or Registered Nurse (RN) certification, preferred An equivalent combination of education and experience in related field, required Compassion for and desire to work with the elderly, required
