As a result, Fountain House has created a comprehensive Community System of Care. This includes Clubhouses in Manhattan, the Bronx, Hollywood, California and Care Management, Home and Community-Based Services and a large housing program ranging from 24 hour supervised residences to scattered site supported apartments. We are committed to reducing social isolation, advocating for mental health policy change, and driving solutions that empower our members. This new and exciting position is in response to a new Medicaid benefit called Health Related Social Needs (HRSN) and the creation of Social Care Networks (SCN) throughout York State. Since this is a new benefit in NYS and a new role at Fountain House, there is great potential for innovation, system creations, and growth. This position will also have external-facing opportunities in the larger behavior health systems in NYC. This position will be based in the Wellness Unit of Fountain House Clubhouse and responsible for identifying those served within the Clubhouses and other Fountain House programs who have Medicaid, ensuring those individuals (members) are screened for eligibility and navigated to the appropriate health-related social services. Primary to this role is to coordinate with Fountain House Units and Residence staff to ensure all member’s needs are met and the support is documented into the SCN data portals depending on the borough one resides. $30.58 per hour Monday to Friday 9am to 5pm Fully onsite Requirements
ESSENTIAL DUTIES AND RESPONSIBILITIES
Complete extensive training on Social Care Networks and Health-Related Social Needs regarding screening, navigating, providing HRSN services, and documentation requirements on designated data platforms such as Unite Us and Find Help.
Assist in creating systems to identify individuals’ Medicaid status, screen all eligible members, navigate members to Health-Related Social Needs both to Fountain House services as well as to outside organizations participating in the Social Care Network.
Ensure all HRSN screening and navigation conducted both in person and over the phone is completed in a timely manner adhering to strict deadlines.
Document all engagements in platforms as designated and required by the larger Social Care Network.
Track work, gather data points, and report findings needed to evaluate impact to the organization.
Coordinate with direct services staff throughout the organization to ensure their members are engaged in the new services.
Base work and systems out of the Wellness Unit and ensure opportunities for engagement.
Attend Wellness Unit meetings on a daily basis to inform others of your work, incorporate the work into the Unit, and engage members in your work in keeping with the Clubhouse model.
Educate the wider organization about
HRSN/SCN
opportunities and any changes in the systems.
Attend conferences, seminars, workshops, trainings, webinars, and meetings in the effort to gather all up-to-date and relevant information that leads to sound and strategic advisement and decision making.
Perform additional related duties as assigned by supervisor.
Maintain compliance with all company policies and procedures.
REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES
Demonstrated competency in written, verbal and computational skills to present and document records in accordance with program standards.?
Excellent interpersonal and conversational skills required.? ?
Excellent computer proficiency (MS Office – Word, Excel, and Outlook) and ability to learn and navigate new portal systems
Excellent and demonstrated organizational skills required
Must be able to work under pressure and meet deadlines, while maintaining a positive attitude and providing exemplary supports and services
Ability to work independently and to conduct assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices
REQUIRED AND PREFERRED EDUCATION, EXPERIENCE, AND CREDENTIALS
Graduate degree in a mental health, public health, or human services related field preferred but will consider an Undergraduate degree and an equivalent combination of training and experience.
Minimum of 2 years’ experience in mental health work.
Clubhouse model experience a major plus.
Bilingual in Spanish preferred
