Monday – Friday, 8a-4pJob Summary The Care Coordinator provides comprehensive,client-centered care coordination by planning, facilitating,documenting, and reporting integrated services across public andprivate providers. This role ensures accurate data collection andreporting of each client’s progress and facilitates referrals incompliance with Coordinated Entry System standards. The CareCoordinator also manages referrals for temporary housing, rapidrehousing, and permanent supportive housing (PSH) within theContinuum of Care framework. As a GBHI Housing Care Coordinator,you will support individuals experiencing homelessness by helpingthem secure stable housing and improve their overall well-being.

Your work will involve direct engagement with clients in varioussettings, including temporary housing, shelters, parks, or even inyour vehicle when necessary. In this role, you will provide casemanagement services, assist with housing placement, and offercontinued support to help clients maintain their housing. Successin this position requires adaptability, strong problem-solvingskills, and a hands-on approach to addressing the complexchallenges your clients face. You may also be responsible fortransporting clients to important appointments, job interviews, andother essential services. Since one agency vehicle is shared amongfour employees, you will need to use your personal vehicle fortransportation at times.

Required Qualifications Education:

Associate degree in Health or Human Services.

Experience:

Relevantexperience in recovery supports, community resources, housing,employment, and other professional and non-professional services.

Experience working with individuals with substance use disorders,mental health diagnoses, and chronic conditions preferred.

Technical Skills:

Proficient in Internet navigation, MicrosoftOffice (Outlook, Word, Excel), and electronic health records.

OtherRequirements:

Valid NYS Driver’s License and personal vehicle withvalid registration and inspection. Demonstrated knowledge ofhomelessness services or comparable human services programspreferred.
