Discharge Planner 2.7 2.7 out of 5 stars 81 Louden Avenue, Amityville, NY 11701 A Discharge Planner at Brunswick Hospital Center is an imperative role to assist with and work hand in hand with the Social Work Department on conducting in-person assessments of patients while in our care to better understand their needs and refer to appropriate services prior to discharge. The Discharge Planner will review medical record as indicated to maintain awareness of patient’s status and serves as a member of the interdisciplinary care team. The Discharge Planner will also carry out assignments requested by the Director of Social Work and Assistant Director of Social Work follow-up with patients, physicians, family members, guardians, etc. as indicated. Responsibilities to include, but not limited to: Work directly with the Social Worker assigned to determine and assess the needs of the patient Clinical packets creation Refer to various support service entities such as Care Management, SPOA, SPA, HRA, etc. Refer to various outpatient or inpatient treatment entities such as OPMH clinics, Inpatient rehabs, Partial Hospitalization Programs, etc. Follow up calls to determine aftercare compliance of discharged patients Faxing discharge packets to aftercare providers Make transportation arrangements for discharging patients Maintain the organization of the office including keeping paper filled in fax/copier, purgatory items taken, organizing supplies, ordering office supplies, organizing fax transmission reports, re-faxing as needed. Check Social Workers messages, as needed, and prioritizing them Other duties as assigned

Minimum Degree Needed:

Master’s in

Social Work Experience:

1-2 years in the field preferred, but not required. Licensed not required for this position. Preferred experience in discharge planning and care coordination.
