Located in Malvern, Pa, the Care Coordinator at Devereux Children’s Behavioral Health Center (DCBHC) will impact the individuals we serve by acting as an advocate and providing problem resolution and team collaboration for clients by obtaining needed after care services for education, medical and mental health services. You will work closely with primary clinicians to ensure high quality service delivery for children and adolescents in an acute mental health hospital setting. Welcome families/guardians to the Devereux Family, relaying expectations for family engagement/involvement while child is in care Communicate with families/guardians the treatment and services being provided to their child
Participates in client treatment team meetings and reports on family engagement, supervised family visitation, family court, and care coordination activities that facilitate current treatment and discharge planning

DCBHC, the only free standing children’s psychiatric hospital in Chester County, Pennsylvania, employs a team of acute care professionals dedicated to meeting the needs of children and adolescents ages 5-18, and their families, in acute mental health crisis. Many of the individuals we serve are from southeast Pennsylvania, but DCBHC has served children and adolescents from other regions of Pennsylvania as well as neighboring states. Devereux Pennsylvania Children’s Services (DPACS) center serves children, adolescents and young adults with autism spectrum disorders, intellectual and developmental disabilities, behavioral, emotional and psychiatric challenges and disorders. Located in West Chester and Malvern Pennsylvania, services provided include psychiatric residential treatment (PRTF), acute inpatient psychiatric hospital (AIP), assessment services, community-based mental health services, community-based employment training and social skills training, and approved private school educational services to male and female children and adolescents (6-21 years of age) with diagnoses of intellectual disabilities, autism spectrum disorders, concomitant psychiatric and behavioral challenges often associated with experiencing some level of trauma in their lives. #Sponsored Qualifications

EDUCATION

Bachelor’s degree in Social Work or related discipline required.

EXPERIENCE

Six months experience in Social Service preferred. Proficient computer skills, mastery of Microsoft Office Applications, experience with data collection/analyzing platforms (Radar, Pro-Filer, and/or other internal and external software applications pertinent to the position). Strong organizational skills. Excellent oral and written communication skills. Requires ability to work independently, prioritize, manage multiple tasks simultaneously, and adhere to established guidelines. Valid driver’s license must be maintained

PHYSICAL REQUIREMENTS

Significant amounts of walking, sitting, and standing. Some lifting and/or managing resistance required related to applying physical intervention techniques – up-wards of 75-100 lbs. or more. Some repetitive wrist/hand movements, normal hearing and vision. Significant amount of use of computers and office equipment. Occasional driving of Devereux vehicles for business purposes. Ability to meet 100% mastery of Safe and Positive Approaches (S&PA) techniques. Devereux has a zero-tolerance policy for abuse and maintains policies and procedures to systematically reduce the risk of abuse to occur in our organization.
