We believe in compassionate, coordinated, and convenient care that improves patient outcomes. Job Summary We are seeking a highly organized and compassionate Care Coordinator to join our wound care team in Chattanooga, TN . In this role, you’ll work directly with our Nurse Practitioner to ensure seamless coordination of care for patients receiving wound treatment. The ideal candidate is a Registered Nurse (RN) with wound care experience and strong administrative and communication skills. Responsibilities Care Coordination Coordinate wound care services for patients across multiple care settings Develop and follow individualized care plans in collaboration with providers Maintain continuous communication with healthcare facilities, caregivers, and patients Patient & Family Support Educate patients and families on wound care procedures and expectations Serve as a liaison to address questions and concerns throughout the treatment process Administrative Duties Manage patient intake, documentation, and referrals Ensure orders are processed, sent, and followed up accurately and in a timely manner Handle routine administrative communications (faxes, orders, facility communications) Transitions of Care Coordinate discharges and follow-ups for patients moving between hospital, facility, and home care Ensure accurate hand-offs and continuity of care Team Collaboration Partner with Nurse Practitioner and clinical team to align care goals Participate in team meetings and case reviews Maintain accurate, timely documentation using electronic health records (EHR)

Qualifications Required:

Current RN license in Tennessee (LPN with strong experience may be considered) At least 2-3 years of experience in wound care, care coordination or related fields Strong organizational and communication skills Experience working in skilled nursing, assisted living, home health, or senior care environments Proficiency with EHR and healthcare documentation

Preferred:

Certification in wound care (e.g., WCC, CWCN, CWON) Experience in case management or discharge planning Familiarity with Medicare/Medicaid wound care requirements Working Conditions Full-time position based in Chattanooga, TN Some local travel required Hybrid work environment (remote administrative tasks + in-person facility visits)
