Job Summary:

The Patient Outreach Specialist plays a key role in supporting population health and quality improvement initiatives at Mercy Community Healthcare. This position is responsible for proactive outreach to patients across a range of populations, including those listed on TennCare and other managed care organization (MCO) rosters, patients who are due or overdue for care, and those identified on gaps in care or quality metric reports. Additionally, this role includes reviewing attributed patient lists from MCOs to ensure timely engagement and care coordination. The Patient Outreach Specialist reports directly to the Quality Manager.

Essential Duties and Responsibilities:

Conduct targeted outreach to: Patients listed on TennCare and other MCO rosters Patients attributed to the health center by MCOs Patients who are due or overdue for preventive, chronic disease, or follow-up care Patients appearing on internal gap-in-care or quality tracking reports Patients discharged from hospitals or emergency departments, as identified through the ADT list, to ensure timely follow-up and care transitions Review and monitor attributed patient lists from MCOs, identifying patients in need of outreach or follow-up care. Utilize EHR and data dashboards to track patient care needs and document outreach activities. Coordinate scheduling of appointments and follow-ups in collaboration care teams. Monitor performance on key quality metrics such as HEDIS, UDS, and PCMH to inform outreach priorities. Participate in quality meetings and contribute to strategy for closing care gaps. Ensure compliance with reporting requirements from TennCare and other MCOs. Assist with planning and executing outreach campaigns and quality improvement initiatives. Provide general education and guidance to patients on the importance of preventive care and follow-up. Maintain confidentiality and adhere to all HIPAA and patient privacy policies.

Qualifications:

High school diploma or GED required; Associate’s degree or higher in health or human services preferred. One (1) or more years of experience in healthcare, patient outreach, care coordination, or related roles. Experience working with TennCare and/or managed care organizations preferred. Knowledge of quality measures and care coordination principles (e.g., HEDIS, UDS, PCMH) preferred. Strong written and verbal communication skills. High attention to detail and ability to manage multiple priorities effectively. Proficiency in Microsoft Office (Excel, Word, Outlook) and EHR systems. Bilingual abilities are a plus.

Working Conditions:

Office environment, with potential for hybrid/remote outreach work as needed. Frequent interaction with patients by phone, email, and other platforms. May require occasional evening or weekend hours for specific outreach initiatives or events.

Physical Demands:

Prolonged periods of sitting at a desk and working on a computer. Ability to communicate effectively via phone and in writing.
