Clinical Mentor Home Health 3.4 3.4 out of 5 stars Jackson, MS 39201 The Clinical Mentor supports the company’s training and development objectives through direction, guidance, coaching and support. Responsible for orientating and integrating newly hired field clinicians into the company, agency and role-specific duties utilizing company policies and best practices. Serves as a role model, provides leadership, demonstrates professionalism, and the values of our company. Collaborates with the Preceptor Program Supervisor, Preceptor and local agency leaders to evaluate the new hire’s skills, identify learning gap needs and evaluate training effectiveness. Upholds the mission, vision, and values of the organization through effective precepting and leadership Establishes and maintains positive working relationships with agency management and staff of assigned areas In collaboration with the Preceptor, provides direction, guidance, coaching, and support to new hires during the field training phase of orientation Ensures progression of the new hire’s orientation plan to support the transfer of knowledge, competencies, and practice standards Collaborates with preceptor and agency leadership to identify knowledge gaps and create individualized learning plans to address areas needing improvement. Conducts final visit evaluations for new hires within the assigned region to confirm competency in clinical skills, documentation, regulatory compliance, and the use of organizational tools such as EMRs and mobile technology, ensuring readiness for independent patient visits upon completion of orientation. Acts as mediator to resolve issues between the preceptor and new hire when they are unable to do so on their own Maintains accurate records of orientation and training activities, including use of Competency Assessment/Peer Review and New Hire Weekly Assessment forms Manages workload to allow appropriate time for teaching and learning activities, including formal and informal feedback. Maintains current knowledge of state-specific education requirements for assigned areas Assists with remediation of current staff in the field in coordination with the QA team and agency leadership Acts as a resource and provide support to preceptors, program supervisors, and leadership Willing to travel within assigned region Performs all other duties as assigned Current professional licensure in state of practice Current CPR certification required. Current Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation Minimum 1 year tenure with the company AND has 1 year of prior professional clinical experience in home health service line Current performance appraisal with ratings of meets expectations, above average expectations, and/or exceeds expectations in all areas of responsibility. Proficient in key competencies and standards of practice for their role, including but not limited to: Bag Technique Infection Control Wound Care Medication Reconciliation HHCAPHS Measures Sync time (RN, Therapist, MSW) – 90% If an OASIS clinician, demonstrates OASIS proficiency benchmark score of 85% or greater as per QR team auditing Successfully completes joint shadowing visit and documentation review with designated QA Team Member prior to independently performing in the Clinical Mentor role and annually thereafter Current with all assigned and mandatory learning Experience in working collaboratively across multiple disciplines Proficient in the rules and regulations relevant to Home Health service line Ability to identify competence for tasks, jobs, and roles in Home Health Service line Ability to interpret training needs and determine most appropriate training solutions Ability to work independently Ability to communicate effectively in verbal and written formats
