This position requires organization, accountability, and empathy. We are a full-service, 12-step, and spiritually oriented recovery residences program for men, dedicated to providing guidance and mentorship to individuals in recovery. Our case management team works closely with residents to support their recovery and all other aspects of their lives, including career development, finances, mental health, health, and wellness. Each resident participating in case management services will receive 3-9 hours of direct support per week, based on their needs. Case managers will be mentored by a licensed clinician who oversees our case management team. Job Expectations :
1. Admission Process Contact new clients within 48 hours of intake via phone or in-person. Schedule case management assessments and make introductions. 2. Assessment Complete case management assessments within 3 days of intake, with an operations staff member present. Review assessments monthly to track progress and update goals. Document all assessments in the Oathtrack system. 3. Goal-Setting & Individualized Growth Plans Collaborate with clients and staff to complete assessments and growth plans. Create SMART goals (Specific, Measurable, Attainable, Relevant, Time-bound). Set weekly goals based on client objectives. Update progress weekly in the resident goals Google document. 4. Documentation Submit session notes within 24 hours (including phone calls, asynchronous work, and updates). Record notes in both Oathtrack and WhatsApp. Immediately report emergencies, behavioral concerns, or drug test failures to the Program Director/CEO. 5. Communication Send weekly email updates to appropriate contacts (e.g., family, payee, clinical staff). Complete all notes within required timeframes to remain in good standing. 6. Psychiatric & Medication Support Obtain Releases of Information (ROI) for psychiatric care at the first appointment. Stay informed about client medications and appointments. Be present during pickup of controlled substances; communicate changes to operations staff. All psychiatric referrals require approval from the Founder and Director of Outreach. 7. Clinical/Therapy Support Obtain ROIs for therapeutic care during initial appointments. Keep up with clients’ therapy schedules. Communicate client concerns directly with clinicians as needed. 8. Transporting Residents Maintain a valid driver’s license and active insurance. Secure liability insurance prior to transporting clients. 9. Crisis Management Respond to crises, including relapse, missed deadlines, or mental health emergencies, with urgency and professionalism. 10. Task Management (Todoist) Use Todoist to track client needs, coordinate with the operations team, and manage assigned tasks. 11. Time Tracking Accurately clock in and out for all client-related work. Review and submit timecards weekly. Notify the VP of any timekeeping issues. 12. Team Communication Share relevant updates and concerns with your supervisor. Respond to client-related information in WhatsApp in a timely and appropriate manner. 13. Supervision Attend weekly one-on-one supervision sessions. 14. Case Management Meetings Participate in case management meetings, prepared to discuss client progress and challenges. Notify your supervisor 24 hours in advance if unable to attend, and reschedule accordingly.
Qualifications :
Valid driver’s license and active HBSO liability insurance policy (for transporting clients). Ability to work with a diverse group of residents and support their recovery needs.
