Position Summary:

The Case Manager serves as a vital part of the clinical team, responsible for supporting clients through their treatment journey from admission to discharge. This includes assessing client needs, coordinating services, maintaining accurate documentation, and collaborating with multidisciplinary team members to ensure individualized, comprehensive care.

Key Responsibilities:

Conduct biopsychosocial assessments upon admission and develop individualized service plans. Coordinate discharge planning, including aftercare referrals to outpatient treatment, sober living, or mental health services. Collaborate with therapists, medical providers, and support staff to address client needs and ensure continuity of care. Communicate regularly with family members or approved contacts, as appropriate, regarding treatment progress and discharge planning. Maintain detailed and timely documentation in compliance with state regulations and facility standards. Assist clients in obtaining identification, insurance, employment resources, and transportation services as needed. Participate in daily clinical meetings and case reviews. Ensure client confidentiality and maintain HIPAA compliance.

Qualifications:

Bachelor’s degree in social work, psychology, or a related field required (Master’s preferred). Minimum of 1 year experience in substance abuse treatment, behavioral health, or social services. Familiarity with levels of care including detox, residential, and outpatient treatment. Strong organizational and communication skills. Ability to manage a caseload and meet deadlines in a fast-paced environment. Knowledge of community resources, discharge planning, and case management best practices. CPR/First Aid certification (or ability to obtain upon hire).
