Full Job Description Now Hiring:
Clinical Case Manager – On-Site in Framingham, MA $5,000 Sign-On Bonus | Full-Time Opportunity We are seeking a skilled and compassionate Clinical Case Manager for a full-time, on-site role in Framingham. This is a rewarding opportunity to work within a multidisciplinary team, guiding patients through the continuum of care and ensuring optimal clinical outcomes from admission through discharge. This position plays a vital role in coordinating resources, managing care plans, and supporting both patients and families through the rehabilitation process.
Key Responsibilities:
Collaborate with physicians, therapists, nursing staff, and other disciplines to create and manage comprehensive care plans Lead the coordination of discharge planning, ensuring timely transitions and appropriate post-discharge services Facilitate weekly interdisciplinary team conferences to review and adjust patient treatment plans Monitor patient progress and satisfaction, including quality, service expectations, and payor requirements Serve as the primary liaison for patients and families, ensuring open communication and education throughout the care journey Maintain regulatory compliance and complete required documentation, assessments, and case management reports Understand and manage payor guidelines, exclusions, and recertification timelines Participate in utilization review activities and quality improvement initiatives Provide timely referrals and ensure accurate discharge documentation Participate in the on-call case management schedule as needed
Qualifications:
Must be a licensed healthcare professional (RN, OT, PT, ST, Social Worker, or Rehab Counselor) eligible to conduct independent assessments within scope of practice Active licensure in the state is required if applicable to your discipline Must be eligible for CCM® or ACM™ certification upon hire; certification required within 2 years of placement into the role
For RNs:
Associate Degree in Nursing required; BSN preferred For non-nursing professionals: Bachelor’s degree required; graduate degree preferred At least 2 years of rehabilitation experience is preferred
