• Lead weeklytreatment conferences and support transitions post-discharge
• Coordinate with patients, families, and payors to ensure smoothcommunication and appropriate services
• Monitor quality metrics,regulatory compliance, and plan-of-care effectiveness
• Completetimely documentation and contribute to utilization review processes
• Facilitate family meetings, resource planning, and referralcoordination Job Details
• Active licensure in your clinicaldiscipline (RN, LCSW, OT, PT, ST, etc.)
• Minimum Associate Degreein Nursing or Bachelor’s in a healthcare field (graduate degreepreferred)
• Must be eligible for CCM® or ACM™ certification(required within 2 years of hire)
• Experience in rehabilitation oracute care settings preferred
