Full-Time, Monday-Friday, 8:00 AM-5:00 PM, with weekend rotation as needed All About People is seeking a qualified and compassionate Clinical Case Manager to support a top-tier inpatient rehabilitation hospital in Framingham, Massachusetts . This is an excellent opportunity for a healthcare professional looking to grow their career and make a meaningful impact on patient outcomes. Sign-on bonus of $5,000 available for the Framingham location.

Key Responsibilities:

Coordinate individualized patient care from admission through discharge Collaborate with interdisciplinary teams to develop and execute treatment plans Monitor quality of care, service timelines, and resource use Participate in weekly team conferences to review and modify care plans Conduct patient and family education on the rehabilitation process Facilitate discharge planning and ensure continuity of care Maintain accurate and timely documentation Understand and apply knowledge of payor requirements and clinical regulations Attend team and utilization review meetings as needed

Qualifications:

Must hold a current license or certification in a healthcare discipline such as RN, Social Work, OT, PT, ST, or Rehabilitation Counseling Must be eligible for CCM® or ACM™ certification upon entry or obtain within two years RN candidates must hold at least an Associate Degree in Nursing (BSN preferred) Other licensed professionals must hold a minimum of a bachelor’s degree (graduate degree preferred) At least 2 years of experience in rehabilitation or case management preferred
