The Case Manager is responsible for creating a safe, comfortable and supportive environment for the patients participating in the program. He/she will provide group therapy as ordered by the Program Director & Clinical Supervisor.

Clinical Dept Position Responsibilities Monitors each patient throughout the daily schedule. Assures Compliance with expectations of attendance at trainings. (Including mandatory Relias Trainings) Attends all clinical meetings including treatment team (rounds) Completes all paperwork pertaining to patient, including but not limited to Bio-psychosocial, Treatment plan, Assessments etc. in a concise timely manner. Enforces program rules and expectations and reports infractions to Program Director or Director of Clinical Services. Monitors patient conformance with program schedule, reports problems and informs supervisor. Makes daily assignment checks and discusses results with each patient. Monitors patient participation and reports problems to supervisor. Resolves conflicts through general and supportive advice. Participates in patient recreation, leisure, and weekend activities as scheduled. Provides case management services. Prepares educational and motivational lectures and groups. Understands and has a working knowledge of 42 CFR Part 2 in accordance to Evoke Wellness and Its patients and: Maintains the confidentiality of all patient and facility activities. Complies with Policies and Procedures. Accepts and utilizes supervision Demonstrates willingness to accept responsibility Completes all tasks with concern for work quality Being sensitive to Cultural Issues of a patient based on their job position Reporting Suicidality/Homicide of a patient based to their job position. Ability of competency and understanding to Addition based on their job position. Reporting and responding to issues of Trauma of a patient based on their job position. Reporting and responding to issues of Dual

  • Diagnosis issues of a patient based on their job position.

Understanding of Clinical Progress Note writing in DAP format. Competencies Demonstrates Proficiency in Communication, observation & Written skills. Knowledge of Drug-Free Workplace Policies. Knowledge of Workplace Violence. Knowledge of Corporate Integrity & Ethics Knowledge of addictions. Ability to interact with a multi-disciplinary team to assist in facets of the recovery program relating to physical health. Knowledge of the 12-Step Recovery Program. Knowledge and competency in problem solving, stress management, ethics and team building. Knowledge of alcohol and other drugs that includes: Alcohol and addictive drugs and their physical, emotional, intellectual, and spiritual impact on the individual. o Alcohol and addictive drugs and their impact on the family. Ability to determine if a medical emergency exists and to take appropriate action, when necessary. Knowledge of emergency procedures used in case of alcohol and/or drug overdose. Knowledge of First Aid and CPR. Knowledge of Federal, State and Municipal laws and/or regulations that regulate the treatment of alcoholism and drug addiction, including: 42 CFR Part 2 Confidentiality HIPPA, Patient Rights, Professional Code of Ethics, Discrimination, Flexibility to adapt to schedule changes and assumption of responsibilities not delineated in the job description which are related to work as a member of an addiction treatment team. Knowledge of Microsoft Office Programs (i.e. Excel, Word, and Outlook, etc.) and Google Suite (i.e. Docs, Sheets, Slides, etc.) Education & Work Experience High-School/GED Six-month or more years of experience in the Human Services field or a related field. CADC or LADC 2 Certification perferred (will help work towards obtaining this while employed with Evoke Wellness if one desires) Schedule Sunday
