Clinical Case Managers (Full-Time & PRN) – Braintree, MA We’re actively seeking experienced Clinical Case Managers for both full-time and PRN roles at our rehabilitation facility in Braintree. This is a great opportunity for compassionate professionals who are passionate about care coordination and ready to make a meaningful difference in the patient experience. Why This Role? This is not just a case management position—it’s a key clinical role on the front lines of patient rehabilitation. You’ll guide patients and their families through the continuum of care, coordinating treatment plans, ensuring safe discharges, and advocating for the best possible outcomes in collaboration with a highly skilled interdisciplinary team.

Position Highlights:

Coordinate all aspects of patient care from admission to post-discharge Facilitate treatment planning, patient/family communication, and discharge readiness Lead weekly interdisciplinary team conferences and update care plans accordingly Monitor patient progress, payor requirements, and quality indicators Serve as a resource and advocate for patients and families Manage documentation and compliance requirements

Qualifications:

Must be a licensed clinical professional (RN, OT, PT, ST, or Rehab Counselor) Active state license required if applicable to your discipline Associate Degree in Nursing required (BSN preferred); bachelor’s degree required for non-nursing disciplines Must meet eligibility for CCM® or ACM™ certification upon hire or be willing to obtain within two years Prior experience in healthcare case management is preferred
