• 41/hr + mileage reimbursement Benefits are available to full-time employees after 90 days of employment A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates Care Manager Highlights This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs The required availability for this position is Monday-Friday, 8:00 AM•5:00 PM (remote with 80% local travel) Care Manager Responsibilities Manage a caseload of healthcare members with long-term care needs Conduct monthly and quarterly member contacts Complete member assessments, documentation, and care plans Perform in-person visits and telephonic follow-ups with members, caregivers, and providers Authorize and coordinate referrals and service delivery Identify care gaps and functional deficiencies in care plans Collaborate with discharge planners and other care teams to ensure seamless care transitions Assist members with grievances, complaints, and appeals Integrate informal or voluntary services into care planning Travel up to 80% locally to meet with members (remote otherwise) Care Manager Requirements Licensed Clinical Social Worker (LCSW) or Registered Nurse (RN) license Valid driver’s license 2+ years of care management experience Experience in physical health, care management, utilization management, or home health High School Diploma or GED Attendance is mandatory for the first 90 days Preferred Qualifications 4-6+ years of care management experience Experience with long-term care, discharge planning, and/or home health Familiarity with electronic medical health records Proficiency in Microsoft Office If you think this position is a good fit for you, please reach out to me•feel free to call, e-mail, or apply to this posting!
