Job Profile Summary:
Provide intensive case management & therapy services to ensure the quality of services remain appropriate to the level of need. Serves as a liaison to the treatment team members and serves as a representative on behalf of the member and LifeConnections. Conduct comprehensive bio-psychosocial assessments and complete treatment plans for assigned members within the first 30 days of intake. Actively participate as a member of the treatment team, keeping all members informed of case status and progress. Provide monthly reports with recommendations about case goals for treatment plans. Providing appropriate referrals for youth requiring additional or alternative services. Complete intakes and discharge summaries for all assigned cases. Complete quarterly treatment plan reviews and obtain approval by the prescribing practitioner/clinical supervisor. Conduct sessions and in-home visits to provide therapeutic services needed to assist child with achieving treatment plan goals. Attend all treatment plan review meetings, including school meetings, court hearings, and scheduled therapy sessions as a representative of LifeConnections and the families supported by LifeConnections. Assist with re-licensing home assessments as needed. Conduct day to day case management activities and coordination of services for each youth on caseload. Collect and complete necessary paperwork as it pertains to the child’s case file. Respond to crisis and provide support when needed. Provide supportive activities that assist the youth and family in goal achievement as it relates to identified goals.
