Elevated Health NYC is a full-service physical therapy and rehabilitation practice with 3 office locations in the NYC area. Our mission is to help as many people as possible live happy, healthy pain-free lives. Being one of the only offices in the NYC area that accepts Federal Workers’ Compensation Insurance (OWCP-DOL), we pride ourselves on our very high success rate in getting our patients’ cases approved and getting them the care and compensation they deserve. A huge part of our success is due to our strong and collaborative Case Management Team. We are currently looking for a part time, hybrid/remote Medical Case Manager to join our Case Management team. We’ve added 2 new locations to our practice in just the last year and will be continuing to expand in the coming year so that we can better serve more patients in need, therefore we will need more people on our team as we continue to grow. Role Description This is a part-time hybrid/remote role for a Medical Case Manager. This person will be responsible for speaking to current patients on a daily basis, intaking new patients, collecting medical records, developing patients’ workers’ compensation cases, and advocating for our patients. They will collaborate with other Case Managers, medical professionals, DOL Claims Examiners, but most importantly, our patients, to ensure quality care and attention. The right person for this job will have exceptional organizational and managerial skills with a willingness to provide excellent customer service to all of our patients. The applicant should appreciate and thrive in a team setting where the focus is always on the patient. (

• Please note- this is not a Social Work position) Qualifications Strong interpersonal and communication skills Ability to prioritize tasks and work independently Willingness to wear many hats and support where needed Experience in case management or healthcare coordination Previous experience in the medical field is a plus Bachelors Degree required
