MINIMUM QUALIFICATIONS

Certified Peer Support Specialists OR high school diploma, 1-year experience working in the alcohol and drug treatment field, and must be actively pursuing Certified Peer Support Specialists certification. Ability to communicate orally and in writing.

GENERAL RESPONSIBILITIES

Provides direct case management services to clients. This includes, but is not limited to, conducting individual planning sessions, assisting the client in continuing care placement. The peer support case manager will provide crisis management, relapse prevention skills to clients in the peer support community The case manager will work closely with the community housing coordinator to assure all peer support community clients have document case management, drug screen, and appropriate contacts with clients referral sources.

KEY DUTIES
  • With the client and other multi-disciplinary team members, evaluates the client, develops an individualized case management assessment and monitors the clients progress, documenting appropriately as they progress in the case management assessment.
  • Conducts Psycho Education groups as assigned by the community housing coordinator
  • Assists the client in planning for aftercare, making the appropriate referrals.
  • Maintains communications between their program and other Mending Hearts programs ( as well as other agencies to which they are making referrals) to ensure continuity of client care.
  • Assists the community housing coordinator in ensuring agency compliance with licensure and other regulations imposed by the state, insurance companies, contracts, etc.
  • Establishes and maintains contact as requested by the referral source of each client assigned, providing appropriate updates for them.
  • Participates in community activities and trainings as assigned.
  • Performs other duties as requested by Clinical Director.
  • Maintains appropriate records to ensure housing fees are paid within Mending Hearts timelines.
  • Provides back up for all transportation of clients in the peer support community.
  • Assure all housing contracts are completed and uploaded to clients EMR
  • Any other specific tasks or projects as assigned by community housing coordinator.
