Walk-In Navigator/Medical Case Manager Choice Health Network 1925 Ailor Avenue, Knoxville, TN 37921

POSITION SUMMARY:

Choice Health Network is seeking a compassionate and adaptable Walk-In Navigator to provide direct support and care coordination for clients accessing our services, which include infectious disease treatment, harm reduction/syringe exchange, case management, and HIV prevention. Core responsibilities include Ryan White and ARTAS case management, linkage to medical and supportive services, and community outreach. The Walk-In Navigator reports to the Client Services Team and works closely with harm reduction, medical, and prevention staff to ensure that clients who face barriers to care receive seamless, client-centered support. This position also involves achieving goals of the Antiretroviral Treatment and Access to Services (ARTAS) program — a strengths-based case management model to link individuals recently diagnosed with HIV to medical care.

KEY RESPONSIBILITIES

  • Provide support for people living with HIV who are newly diagnosed and/or experiencing significant barriers to treatment, including ARTAS and Ryan White enrollment and case management services focusing on linkage to medical care.
  • Identify client strengths and address barriers to care and medication adherence.
  • Enroll eligible clients in the Ryan White Part B program and maintain accurate documentation per program standards.
  • Offer supportive services and develop care plans based on client needs and goals.
  • Provide referrals and resource navigation for social services.
  • Conduct ARTAS follow-up after medical visits to support clients’ care plan understanding and adherence.
  • Perform thorough suicide risk assessments and facilitate referrals to mental health services or emergency care as needed.
  • Arrange client transportation to medical appointments as necessary.
  • Conduct medical intakes and needs assessments for clinic patients.
  • Consistently communicate with internal teams to support client access and retention.
  • Maintain thorough documentation, data entry, and client records.
  • Respond promptly to calls, emails, and other communications.
  • Support mobile harm reduction outreach in East TN (syringe services and testing).
  • Participate in community outreach and events.
