7K a year 15 hours ago Qualifications RN License Mid-level 3 years Physical Therapy License Discharge planning Acute care CRT Nurse Licensure Compact (NLC) Full Job Description Inpatient Clinical Rehab Liaison A Brief Overview The Inpatient Clinical Rehab Liaison serves as liaison to coordinate and link hospital resources, acts as interface with third party payors/home health, coordinates research projects and provides adequate follow-up and rehabilitation for patient; also serves as clinical liaison to identify compliance/quality improvement problems. Provides professional clinical services according to established standards and practices. What you will do Attends patient rounds discussing discharge planning with medical staff and case managers Collects functional and medical information within the pre admission screening assessment within time constraints Discusses SNF and IRF differences fluently to patients/caregivers/physicians Maintains follow up communication with discharge planner/case manager/patient/family/hospital departments regarding all aspects of transition to post-acute care Supports and grow the organization by representing and promoting the IRF clinical services and programs to internal physicians/case managers/social workers and patients/families Serves as the voice of the organization in the medical community to increase patient referrals and secure new business. Provides presentations on new and existing services Interacts and communicates in a customer responsive and professional manner. Works collaboratively and coordinates activities with hospital/medical staff and community agencies to integrate and implement activities. Educate patients about treatment options and answer any questions. Act as a liaison between doctor and patient, collaborating with care team to develop and implement a treatment plan. Provide clinical resources and address barriers to care. Document all customer contact and information. Supervise implementation, advocate for the customer’s needs, and communicate with client. Identify, prioritize, and reach out to new business categories, partners and opportunities. Has a broad knowledge of product/service lines, and an in-depth knowledge of specific areas. Prepares and maintains required reports, records, and files. Participates on various clinical and hospital committees as requested. This position requires certification, licensure, or registration within the clinical discipline. Qualifications Graduate of a RN, Rehab (PT, OT, Speech) or Respiratory Therapist program Required RN

Rehab Admin Schedule:

Full-time, Day shift w/rotation, 8:00-4:30
