SUMMARY:
The Friends For All Corporation Non-Medical Case Manager (NMCM) works within the Ryan White Program to provide support and services to individuals living with
HIV/AIDS.
The NMCM assists clients in accessing a range of social services, including housing, food assistance, transportation, and other supportive services to enhance their quality of life. The NMCM plays a key role in reducing barriers to care, ensuring clients are connected to community resources, and promoting adherence to HIV treatment plans.
ESSENTIAL DUTIES AND RESPONSIBILITIES

Screening:
Collaborate and Consult with Clinical Staff to identify current clients who need medical case management services. Work with agencies in the
HIV/AIDS
service system to engage individuals infected with HIV. Respond within 24 hours to requests for non-medical case management services. Schedule a screening time within two (2) days of the initial request. Perform initial intake interviews to assess eligibility for Ryan White services and Medical Case Management. Ensure compliance with confidentiality and HIPAA regulations.
Orientation and Assessment:
Work with clients to develop a patient-centered care plan which includes realistic, measurable, and mutually acceptable goals that are based on the results of the assessment. Identify action steps needed to achieve each goal, including target dates for accomplishment of the stated goals. Document progress made towards goals by entering case notes in the client database system. Identify referrals made to other providers/services in connection with action steps. Obtain client and case manager’s signature on patient-centered care plan. Copy plans and give them to client and file original in client’s file.
Referral and Case Coordination:
Be knowledgeable of Friends For All services and referral processes. Conduct comprehensive needs assessments for clients to identify social service needs. Collaborate with medical case managers, healthcare providers, and other support services to coordinate care. Attend and participate in Interdisciplinary meetings as scheduled. Assist clients in accessing community-based services such as housing, transportation, food, and legal services. Complete all documentation to show evidence of the time, date, place and description of each case management service. Provide follow-up and reassessments to ensure that client needs are met. Educate clients about available benefits and services, including housing programs, disability assistance, and employment resources. Advocate on behalf of clients to ensure they receive appropriate services and benefits. Assist clients with completing applications for public assistance programs, such as SNAP, Medicaid, and housing assistance. Build and maintain relationships with local community service providers to facilitate referrals and connections for clients. Provide education and support to clients on topics such as health literacy, financial management, and housing stability. Maintain ongoing contact with referral sources and follow up to ensure implementation.
Community Engagement:
Identify and contact community-based organizations, primary care providers, housing services and other needed providers to establish referral agreements and to coordinate collaborative efforts with clients. Attend scheduled Non-Medical Case Management meetings. Become a Healthcare Marketplace Certified Applications Counselor to assist clients in selecting a Marketplace health plan, if needed. CAC certification must be maintained during employment. Maintain ongoing relationships with community partners and support services organizations establish and maintain a seamless referral process.
Data Entry, File/Chart Management:
Maintain accurate and up-to-date client records in compliance with Ryan White Program standards and funding requirements. Document all services and referrals provided to clients in electronic health records. Case Note Documentation Initial assessment of supportive service’s needs. Develop a comprehensive service plan within 30 calendar days of the assessment that assists the client in establishing support services. Update comprehensive service plan bi-annually with the consumer within 6 months of intake/assessment and/or reassessment. Periodic re-evaluation and adaptation of the plan should be implemented over the life of the consumer prior to 6 months are encouraged as needed
ADDITIONAL DUTIES AND RESPONSIBILITIES
Regular and predictable punctuality and attendance is required. Return all calls, emails and text in accordance with the agency’s voicemail and return call policy. Participate in weekly or biweekly coaching sessions with your supervisor. Complete Performance Reviews and
SMART GOALS.
Participate in All Staff Meetings, In-Service Days and Mandatory Trainings. May need to travel in personal vehicle to meet staff, clients, or community partners at FFA locations or at other locations to meet the needs of the position. Duties and responsibilities may be added, deleted or changed at any time at the discretion of management, formally or informally, either verbally or in writing. Develop relationships with community partners to coordinate care and establish a referral network. Monitor MCM inbox, OhMD, and teams for MCM referrals/request. Ensure these requests are forwarded to appropriate team members and/or addressed in a timely manner.

QUALIFICATION REQUIREMENTS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. High School Diploma or General Equivalency Diploma (GED) Minimum of 2 years’ experience working with people living with HIV/AIDS and/or working in HIV prevention preferred Good strategic/critical thinker, self-starter, goal-oriented Excellent verbal and written communications. Ability to operate with purpose, urgency and accuracy in a fast-paced, deadline-driven environment – with particular attention to detail and organization Ability to work with a diverse client population Strong relationship-building both with team and clients (internal and external) Ability to follow verbal and written instructions.
Computer skills:
ability to efficiently navigate the Internet and input data into applicable systems; familiarity with MS Office 365, especially Outlook and SharePoint, Adobe, TEAMS and Zoom Ability to define problems, collect data, establish facts, and draw valid conclusions Maintain confidential information and adhere to all Friends For All policies and procedures regarding sharing of financial information and Protected Health Information (PHI).

Must have a valid State Driver’s License, provide proof of automobile insurance coverage to employer at all times and have an automobile to be used daily.
